ALLY’S BOXING BOOTCAMP, LLC (DBA ALLY'S BOXING HUB) MEMBERSHIP TERMS & WAIVER OF LIABILITY RELEASE FORM 

MEMBERSHIP TERMS

I acknowledge that this agreement is for a membership commitment of 12 consecutive months. In the event that I cannot maintain this commitment, I will be charged a $100.00 cancellation fee.

WAIVER OF LIABILITY RELEASE FORM

In consideration for my participation in this facility’s classes, I, the participant, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. 

I wish to participate in the Ally’s Boxing Hub classes offered by Ally’s Boxing Bootcamp, LLC. As a participant of Ally’s Boxing Hub workouts, I represent and agree as follows:

1. The risk of injury from the activities involved in this program is significant, including but not limited to the potential for permanent paralysis and death, serious injury to internal organs, musculoskeletal injuries and possible impairment to my general health and well-being.

3.I knowingly and freely assume all responsibility for any risk of loss, property damage, or personal injury that may be sustained by me, or any loss or damage to property owned by me, as a result of my use of Ally’s Boxing Hub and Ally’s Boxing Bootcamp, LLC. activities.

4. I acknowledge that I am in good physical condition and do not know of any condition or reason that I should not participate in Ally’s Boxing Hub and or Ally’s Boxing Bootcamp, LLC.

5. I understand that a medical examination to assure myself of physical fitness is desirable, that obtaining such an examination is my own responsibility.

6. I understand you may photograph, video tape or otherwise record me and use such photographs and recordings to promote the facility and Ally’s Boxing Hub and Ally’s Boxing Bootcamp, LLC.,. Including but not limited to brochures, website material, or by displaying the same at the facility or other locations.

7. In signing this release, I acknowledge and represent that I read the forgoing membership terms & waiver of liability release form, understand it and sign it voluntarily as my own free act and deed. I am at least eighteen (18) years of age and fully competent. If under 18, my parent or guardian shall also sign.
